Please provide names and addresses of 2 individuals (not related to you) who can be contacted for a personal reference.  Ideally, one individual should know you through work or some other activity that you may have carried out as a volunteer, or in a professional capacity.  
The 2 individuals must have known you for at least 3 years.

Criminal convictions

Given the potential vulnerability of visitors to Centre 33, it is important that we check whether volunteers have any criminal convictions that might raise doubts about their suitability as volunteers.

We must, therefore, ask if you have any criminal convictions for offences against people of a violent or sexual nature, for offences involving unlawfully supplying controlled drugs or substances or for offences involving theft, dishonesty, or deception. Convictions that are spent (as defined by the Rehabilitation of Offenders Act 1974) need not be revealed.
 

If you have ticked Yes, Centre 33 will not automatically exclude you from volunteer duties.  However, the Centre 33 Trustees will consider your application further and will require additional information before making a decision.

Data Protection
Your details will be stored in electronic format primarily for the purposes of contacting you by telephone/email and they will mainly be used by the Centre 33 Chairman, Secretary and Volunteer Manager.  No personal information will be passed on to third parties unless required by law.  The use of the information by Centre 33 is compliant with the Data Protection Act.
